The Nigerian Air Force (NAF) has officially formed a 12-member Aircraft Accident Investigation Board to meticulously examine the circumstances surrounding the crash of its ATR 42 aircraft, NAF 931. The tragic incident occurred on October 5, 2026, in Igbokoda, Ondo State, resulting in the loss of all passengers and crew on board.

In a press release issued on Tuesday, the NAF stated that the newly constituted board is tasked with determining the precise causes of the accident, establishing the sequence of events leading to it, and proposing recommendations to avert similar occurrences in the future. Air Commodore Ehimen Ejodame, the Director of Public Relations and Information at Headquarters Nigerian Air Force, signed the statement, which also conveyed the directive from the Chief of the Air Staff, Air Marshal Sunday Aneke, for continuous search and recovery operations alongside a comprehensive investigative process.

Air Vice Marshal FO Ankeli has been appointed to preside over the Aircraft Accident Investigation Board, while Air Vice Marshal EOE Ebiowe, who heads the Investigation and Recovery Team, will oversee the broader response efforts. The NAF elaborated that the 12-member panel comprises highly experienced personnel, bringing specialized knowledge in various fields including aviation operations, flight safety, piloting, air traffic control, aircraft engineering, maintenance, and medical services, among other critical technical disciplines.

To further bolster the board's expertise, representatives from the Nigerian Civil Aviation Authority (NCAA) and the Nigerian Safety Investigation Bureau (NSIB) have been included, ensuring the integration of civilian aviation insights. The scope of the board's inquiry will encompass an examination of operational, technical, environmental, human, and organizational factors that may have contributed to the mishap.

The statement emphasized that the board's work will involve a systematic process of examining, documenting, and preserving all relevant evidence to ascertain facts and identify both immediate and underlying causes of the accident. Additionally, the investigation will assess the effectiveness of the emergency response, covering aspects such as search and rescue operations, medical assistance, recovery efforts, and the management of the crash site.

Upon completion of its findings, the NAF expects the board to provide evidence-based recommendations designed to address any identified shortcomings, enhance aviation safety protocols, and minimize the probability of similar accidents. Regarding support for the families of the deceased, the Service has set up a dedicated helpline at its headquarters, specifically within the Office of the Directorate of Veterans Affairs, to offer information and clarifications to recorded next of kin. Administrative arrangements, including meetings with affected families and other support measures, have also been activated.

Acknowledging the public's keen interest, the NAF affirmed its commitment to providing timely and verified updates as the investigation progresses. The Service cautioned against premature conclusions regarding the accident's cause, stating that such judgments would be inappropriate until the investigation is concluded and its findings thoroughly reviewed. Search, recovery, and investigative efforts remain a top priority, with the NAF pledging to implement all appropriate safety recommendations derived from the board's final report.
